The Medicare Modernization Act (MMA) requires entities, whose health care policies include prescription drug coverage, to distribute a CMS notice annually to all Medicare eligible policyholders.  Please see the attached disclosure.

There is NO action for you to take; this is just a disclosure that the school district is required to send out annually. It is strictly for informational purposes only.
